The Sleep Wellness Institute (TSWI) is Wisconsin’s only independent sleep disorders center fully accredited for adults and children by the American Academy of Sleep Medicine. As Wisconsin’s preeminent center for the diagnosis and treatment of all sleep disorders for both adults and children, we provide more testing and treatment options, such as in-lab or in-home testing, and treat all sleep disorders, with a special expertise in diagnosis and treating obstructive sleep apnea.
General Description: Under the general supervision of the Clinical Sleep Care Manager, the hospital-based Sleep Navigator is responsible for fulfilling The Sleep Wellness Institute mission, vision and values; navigating sleep disorder patients in a manner that results in effective care coordination and testing through the continuum and increase treatment compliance to maximize overall patient care experience and outcomes. The Sleep Navigator serves as the first point of contact for referring providers, patients, durable medical equipment (DME) companies, or other key integrated care partners. The navigator will follow patient adherence to treatment and a plan of care in preparation for discharge, monitor re-admissions, and provide ongoing measurable outcomes. Key Responsibilities:
