The Sleep Wellness Institute (TSWI) is Wisconsin’s only independent sleep disorders center fully accredited for adults and children by the American Academy of Sleep Medicine. As Wisconsin’s preeminent center for the diagnosis and treatment of all sleep disorders for both adults and children, we provide more testing and treatment options, such as in-lab or in-home testing, and treat all sleep disorders, with a special expertise in diagnosis and treating obstructive sleep apnea.
At The Sleep Wellness Institute, you work with Board Certified Sleep Doctors, Nurse Practitioners, and Sleep Technicians who are Registered Polysomnographic Technologists (RPSGT) and/or Registered Sleep Technologists. The Sleep Wellness Institute holds the gold standard for its sleep technologists.
Under the general supervision of the Clinical Sleep Care Manager, the hospital-based Sleep Navigator is responsible for fulfilling The Sleep Wellness Institute mission, vision and values; navigating sleep disorder patients in a manner that results in effective care coordination and testing through the continuum and increase treatment compliance to maximize overall patient care experience and outcomes. The Sleep Navigator serves as the first point of contact for referring providers, patients, durable medical equipment (DME) companies, or other key integrated care partners. The navigator will follow patient adherence to treatment and a plan of care in preparation for discharge, monitor re-admissions, and provide ongoing measurable outcomes.
Ensure goals and objectives are met or exceeded by providing effective Sleep Care Navigation
Improves coordination of care and testing and increases treatment compliance to maximize the overall patient care experience and outcomes. Includes but is not limited to partnering with patients and families to ensure understanding of care plans and initiating referral generation, providing timely follow up with patients and providers to successfully coordinate sleep related testing and treatment.
Coordinate treatment equipment delivery between the patient and DME company. Facilitate use of all treatment and triages all care and treatment related questions and needs to the appropriate entity (physician, sleep center, DME, etc.)
Collaborate with hospitalists, respiratory therapists, case management and nursing to identify patients at risk for readmission based on respiratory status of pulmonary diseases, sleep disorders and/or other
Education and Experience:
Associate’s degree or equivalent from a two-year college or technical school; or three months to one year related experience and/or training; or equivalent combination of education and experience, OR
Bachelor’s degree from a four-year college or university; or one to twow years related experience and/or training; or equivalent combination of education and experience.
License, Certifications, Registrations:
Certified or Registered Respiratory Therapist issued by the NBRC and Respiratory Care Practitioner license issued by the State of Wisconsin, OR
Registered Nurse licensed by the state of Wisconsin, OR
Registered Polysomnographic Technologist (RPSGT) with at least one year of experience
BLS Healthcare Provider American Heart Association (BLS) for the Healthcare Provider
Knowledge of the various sleep disorders and treatment options
